All Employees of Capital A Productions who operate company-owned vehicles (COVs) are required by company policy to have a current and valid DOT Medical Certificate, a CURRENT AND VALID Driver's License, and a Motor Vehicle Record check on file with the office. Although not a requirement by the Department of Transportation, Capital A Productions holds its drivers to a higher standard and requires yearly DOT Medical Certificates for all drivers.

• ALL DRIVER(S) ARE REQUIRED TO SUBMIT A COPY OF THEIR DRIVER'S LICENSE TO THE OFFICE UPON RENEWAL. ANY DRIVER WITHOUT A CURRENT COPY OF THEIR DRIVER'S LICENSE ON FILE WITH THE OFFICE IS PROHIBITED FROM DRIVING ANY COMPANY-OWNED VEHICLES.

All driver(s) and employees must be sober and not under the influence of any drugs or alcohol while on the job and or while in control of any company-owned vehicle, including possession of company-owned vehicle keys.

Driver(s) are required to complete a pre-trip and post-trip inspection before the beginning and at the end of their trip. This is required by law and must be completed by each driver.

Below is a list of items to check before and after all trips. Please see the back of the binder for the physical pre-trip/post-trip inspection log. All driver(s) are required by company policy and DOT rules and regulations to fill out and complete an hours of service log (HOS) and a Pre-Trip/Post-Trip Inspection form.

Driver(s) wishing to use an Electronic Log Book are allowed to do so. Capital A Productions uses the Motive Driver app, which is free. Below are links to download the app in both the Google Play Store and the Apple App Store.

PRE-TRIP / POST-TRIP INSPECTION

✓ Lights (Headlights, Parking Lights, Running Lights, Turn Signals, and Brake Lights)

✓ Horn (Air-Horn if equipped, and Truck Horn)

✓ Tires

✓ Brakes (Service-Brake, Parking Brake, and Truck Brakes)

✓ Fluids (Fuel, DEF, Oil, Coolant, Transmission Fluid, and Windshield Wiper Fluid)

✓ Emergency Equipment (Fire Extinguisher, Spare Fuses, and Reflective Triangles)

ACCIDENTS

All drivers should notify the Operations Manager immediately if any out-of-service condition is found with a company-owned vehicle, whether it is owned, rented, or leased. All accidents, no matter how minor, should be reported to the Operations Manager immediately. Failure to report any incidents to the Operations Manager will result in disciplinary action, including removal of driving privileges and being prohibited from operating any company-owned vehicles, whether owned, leased, or rented. 

USE OF COMPANY VEHICLES

All drivers should return all company-owned, leased, and rented vehicles to the dock with a Full Tank of Fuel and DEF, when applicable, unless otherwise directed by the Operations Manager not to.

Driver(s) are expected to clean out vehicles daily. No trash, water bottles, etc., are permitted to be left inside company-owned, leased, or rented vehicles at any time. 

Vehicles requiring any detailing or cleaning should be reported to the Operations Manager so that the vehicle can be scheduled to get cleaned and detailed. Company-Owned, Leased, and Rented vehicles shall be maintained at regular service intervals per company policies and within DOT regulations. 

All vehicles will have a DOT inspection completed each year by an independent company hired by Capital A Productions. 

HOURS OF SERVICE LOG BOOK

All driver(s) are required by company policy and DOT rules and regulations to fill out and complete an hours of service (HOS) log book. Capital A Productions will supply each driver with their own logbook. Logbooks should not be shared between driver(s). Logbooks must be completed in full for each trip. Any employee who drives a company-owned vehicle, whether it is a leased, owned, or rented vehicle, shall have in their possession a copy of their DOT Medical Certificate, Driver’s License, and their own logbook. Any driver(s) who is unsure how to fill out and complete their logbook correctly should contact the Operations Manager for help before driving. Logbooks may be obtained from the office or by contacting or notifying the Operations Manager.

DOT PHYSICALS

Capital A Productions requires anyone who is going to be driving any company-owned, leased, or rented vehicle(s) to have a valid Driver's License, a current DOT Medical Certificate. 

All driver(s) who have a current DOT Medical Certificate should email a digital copy, along with your valid driver's license, to caitlin@capitalaproductions.com, or bring them to the office so Caitlin can make a copy for our records.

Anyone who would like to obtain a DOT Medical Certificate to drive Company-Owned, Leased, or Rented vehicles can do so, and the company will be more than happy to cover the cost. 

Driver(s) can go to any location that is certified to conduct a DOT physical. Concentra Urgent Care in Garden City is a good location that some of the driver's have gone to. 

You can also Google "DOT Physical near me" to find other locations certified to perform DOT Physicals. 

Driver(s): In order to be reimbursed by the company, please bring back the physical copy of the DOT Medical Certificate, Receipt, and your Valid Driver's License, and the company will be happy to reimburse you. 

Once we have a copy of your DOT Medical Certificate, Valid Driver's license on file, we will have you watch the accompanying training video and then get you in the system so you can be scheduled to drive the truck.
